Approximately one time every 6 - 8 weeks. /span/p/lilispanProvides case management services which support a positive treatment outcome/span/lilispanServes as therapist in IOP and continued care groups, as well as provide individual therapy as needed for SUD and/or mental health disorders/span/lilispanDevelop patient-centered treatment plans based on results from the screening tools and bio psychosocial assessment/span/lilispanCollaborates with medical staff and treatment team as a whole/span/lilispanConsults with family members and referral sources in order to coordinate patient care/span/lilispanEnsures adequate case management services are identified and addressed/span/lilispanResponsible for maintaining electronic health records which include documentation of treatment activity and progress/span/lilispanEnsures that patient records are accurate, comprehensive and meets regulatory standards/span/lilispanDevelops a treatment alliance with patients that is mutual, collaborative, and individualized, meeting changing clinical needs throughout treatment /span/lilispanFacilitates addiction treatment groups in the IOP and OP levels of care /span/lilispan data-cPrepares and presents addiction/recovery education presentations to all patients/span/lilispanEngages in case management sessions, including assessments, collaborative treatment goals, discharge planning, partnership with referral sources and/or other providers /span/lilispanWorks with treatment team to meet the co-occurring psychiatric needs of patients /span/lilispan data-cCoordinates care for all appropriate support services, including referral sources, family members, and other support providers/span/lilispan data-cResponsible for maintaining timely and well written documentation that meets state and CARF standards/span/lilispan data-cDemonstrates commitment to excellent patient care, which is measured through clinical outcomes, such as patient retention, attendance, successful program completion, transitioning from IOP to OP, treatment goal attainment, etc./span/lilispan data-cProvide evaluations for substance use disorders using DSM-TR and ASAM level of care criteria/span/lilispan data-cAttends and is prepared for professional meetings, including treatment team/span/lilispan data-cOther duties as assigned/span/li/ul
