Clinical Social Worker - Remote Position

Role Overview

Gotham Enterprises Ltd is seeking a dedicated and compassionate Clinical Social Worker to join our team remotely. This full-time position is based in our Hospice Center, where you'll provide essential support to patients and families during difficult times. Key Responsibilities
• Conduct assessments to identify patients' psychosocial needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide counseling and support to patients and their families.
• Facilitate group therapy sessions and support groups.
• Manage case documentation and maintain patient confidentiality.
• Coordinate with community resources to ensure comprehensive patient care.

Required Qualifications
• Master's degree in Social Work (MSW) from an accredited institution.
• Current state licensure as a Clinical Social Worker.
• Experience in hospice or palliative care settings is preferred.
• Strong communication and interpersonal skills.
