Job Overview

We are seeking an experienced and compassionate Licensed Social Worker to join our team in a remote, full-time capacity. As a Licensed Social Worker at arenaflex, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet the benefit plan and/or health needs of our members. This role requires a strong ability to research available options and resources for members with financial, transportation, safety, and housing needs, and to empower members to make positive lifestyle and medical choices.

Key Responsibilities

• Partner with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs
• Research and identify available options and resources for members with financial, transportation, safety, and housing needs
• Empower members to make positive lifestyle and medical choices through telephonic or electronic exchange
• Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Utilize a collaborative process to meet the needs of members across the entire lifecycle

Qualifications and Education

To be successful in this role, you must have an active and unrestricted LSW, LMSW, LCSW, or LISW license in the state of your residence. You should also have a minimum of 2+ years of experience in medical social work or case management, and a Master's Degree in Social Work. Essential Qualifications

• Active and unrestricted LSW, LMSW, LCSW, or LISW license in state of residence
• 2+ years of experience in medical social work or case management
• Master's Degree in Social Work

• Experience working in a telephonic or remote case management setting
• Knowledge of community resources and social determinants of health
