About the Role:

We are seeking a Housing Case Manager to support individuals in the Supportive Housing program by providing case management, advocacy, and resource navigation. This role is ideal for a dedicated professional passionate about housing stability, harm reduction, and client-centered care.

We operate under a Housing First model, which prioritizes immediate access to stable housing without preconditions, followed by individualized supportive services. This approach is grounded in the belief that housing is a basic human right and serves as the foundation for long-term stability.

As a key team member, you will assist clients with housing applications, goal setting, crisis intervention, and long-term stability planning.

Key Responsibilities:

✅ Client Support & Case Management

• Maintain a caseload of 25-30 clients, meeting weekly to assess needs and support progress toward personal goals.

• Assist clients in accessing mental health, chemical health, medical, and other community resources; wrap around services.

• Develop, implement, and monitor individualized goal plans focused on stability and self-sufficiency.

• Document client progress accurately, including case notes, goal plans, and service coordination.

• Use harm reduction and crisis prevention strategies to keep clients housed.

✅ Housing & Community Engagement

• Collaborate with the Housing Coordinator to guide clients through the housing application process.

• Conduct weekly home visits to ensure apartments are maintained and address any concerns.

• Build positive relationships between landlords, property managers, and clients to support long-term housing retention.

• Organize and lead social and recreational activities that foster community engagement.

✅ Team Collaboration & Professional Development

• Work as part of a team to develop and implement resident programs.

• Attend agency meetings, trainings, and collaborative partner meetings to stay informed on best practices.

• Align with the mission and objectives of Start Today Hennepin.

What We’re Looking For:

✔ Experience: At least 1 year of direct service working with individuals experiencing homelessness, mental health challenges, or chemical health issues.
✔ Education: A Bachelor’s Degree in Social Work or a related field is preferred. Relevant work experience may be considered in place of some education.
✔ Skills: Strong verbal and written communication, ability to work independently and collaboratively, and knowledge of supportive housing resources.
✔ Technology: Proficiency in Microsoft Word, Outlook, and basic office technology (printer, copier, scanner, fax).
✔ Transportation: A valid driver’s license, proof of insurance, and reliable vehicle are required.

Work Environment & Physical Demands:
• This role requires flexibility to work in both office settings and in the field, including frequent travel to client homes.
• Must be able to climb stairs, sit, drive, and occasionally lift up to 15-30 lbs.
