About the Role:

We are seeking a compassionate and skilled Home Care Registered Nurse (RN) to join our team, providing acute care to patients in the comfort of their own homes. As a Home Care RN, you will have the opportunity to make a meaningful impact on the lives of patients and their families, while enjoying a better work-life balance and increased flexibility compared to other settings.

Responsibilities:

  • Conduct nursing assessments, medication reconciliation, safety assessments, and implement standardized protocols
  • Administer prescribed medications, obtain blood samples, insert foley catheters, and provide therapeutic interventions
  • Educate patients on managing their illness, prescribed medications, nursing interventions, and care plans
  • Identify and act on changes in patients' conditions, evaluating and revising care plans as needed
  • Document all assessments, interventions, and care coordination details
  • Participate in regular in-service or special case orientations, on-call rotation, and other duties as assigned

Requirements:

  • Associate of Science in nursing
  • Current RN license, specific to the state(s) you are assigned to work
  • One year of adult acute inpatient medical, or medical/surgical care experience within the last five years
  • Current CPR certification
  • Valid driver's license and reliable transportation

Preferred Qualifications:

  • Bachelor of Science in nursing
  • Experience providing RN services in the home, community, or clinic
  • Adult medical critical care experience
