Role Description

The Manager, Care Model Strategic Initiatives, plays a critical role in advancing key acute escalations workstreams at Oak Street Health. This is an opportunity to lead a team and make a significant business impact by impacting patient outcomes. The Manager, Population Health position will report into the Director, Population Health and be responsible for the below.

Responsibilities:
Play critical role in leveraging data analytics and strategic thinking to identify key insights and trends to drive high-impact initiatives 

Drive the development, dissemination, and maintenance of educational materials and care model tools for field teams

Lead other high-priority organizational initiatives as needed 

Develop materials and lead discussions with relevant organizational leaders

Other duties, as assigned

What we're looking for:
3+ years of consulting or other applicable experience in healthcare / population health

Deep understanding of OSH clinical workflows, care model, and clinical documentation best practices

Strong accountability and discipline against timelines and deliverables

Analytical and data savvy; highly proficient with Microsoft Excel

Champions a collaborative working style, able to partner cross-functionally

Experience in project management, able to organize and prioritize Clinical Documentation initiatives to maximize impact and efficiency

Effective communicator, able to communicate effectively across different audiences and stakeholders 

Ability to develop business cases, ROI analysis, and financial analysis

Passion for excellence, willing to go the extra mile to achieve objectives and ensure consistently high-quality work product

Possess strong problem-solving and critical thinking abilities

Bachelor’s degree required

US work authorization

Pay Range
The typical pay range for this role is:

$66,330.00 - $145,860.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
