Job Title: MDS Coordinator (RN)


Schedule: Full-time

Location: Remote/Travel required as needed

Position Overview: We are seeking a dedicated Registered Nurse to serve as a MDS Coordinator. In this position, you will assist in clinically coordinating all aspects of the MDS assessment and care planning process to ensure the highest quality resident outcomes and full regulatory compliance.

Key Responsibilities: 

Conduct comprehensive clinical admissions assessments as indicated for admissions/readmissions

Coordinate with MDS Director regarding review of care plans as indicated and per regulations.

Establish assessment reference dates (ARD) and ensure timely completion and submission of the MDS within required timeframes

Remain up to date on RAI manual and other regulatory updates

Transmit MDS data to CMS and the state database in compliance with current standards

Obtain and update qualifying diagnoses for residents receiving Medicare Part A services

Coordinate with therapy staff to report progress for Medicare Part A/B residents

Participate in QAPI

Respond to medical emergencies and follow established protocols.

Uphold professional standards, patient confidentiality, and regulatory compliance.

Serve as clinical resource for admissions, and MDS processes for nursing staff

Enter new admissions, readmissions including, but not limited to accurate ICD 10 codes, allergies, physicians, primary/secondary contacts, orders, and other information as indicated.

Qualifications: 

Must be a graduate of an accredited nursing program with a current, active Registered Nurse (RN) license by the State Board of Nursing

CPR Certified

Strong knowledge of CMS regulations, RAI/MDS requirements, and SNF billing compliance

Familiarity with Medicare Part A/B reimbursement and PDPM methodology

Ability to provide compassionate, person-centered care to residents with diverse physical and cognitive needs.

Good collaboration and teamwork skills.

Fast and adaptive problem-solving abilities.

Outstanding communication and interpersonal skills to foster relationships with residents, families, staff.

A passion for providing compassionate, high-quality care to seniors.
