Medication Therapy Management Coordinator

Shift: Monday-Friday, Day shift

Remote

 

The Medication Therapy Management (MTM) Coordinator partners with our Medication Therapy Management (MTM) pharmacists to ensure the department runs smoothly and can care for as many patients as possible.  They will provide both direct patient contact, such as outreach and scheduling, as well as behind-the-scenes support for billing, prior authorizations, and population health metrics.

Responsibilities

Patient Outreach, Scheduling, and Follow-up

Process referrals to the MTM team in utilizing Epic and schedule as needed

Responsible for identifying care gaps for patients, proactively outreach patients to schedule appointments on behalf of the MTM pharmacists and team

Greet patients and check them in for face-to-face appointments as needed

Provide phone support, including accurate messaging and spelling, screening, scheduling appointments, and directing callers to appropriate team members or clinics as needed

Provide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processes

Follow-up with patients between visits to collect, document, and communicate accurate information including medication list changes, blood sugars, and other patient-specific information

Back Office Support

Under supervision of the pharmacist, help complete paperwork such as prior authorizations or copay assistance programs to help patients access affordable medications

Review charts to ensure MTM billing requirements are met

Collaborate with payors to ensure good communication on share goals such as HEDIS metrics

Department Support

Triage patient lists generated from payors or UPH stakeholders to assign to the appropriate pharmacist

Assist with data collection for internal projects

Qualifications

Education: 

High School degree

 

Experience:

At least one (1) year of Healthcare-related experience

 

License(s) /Certification(s):

Valid driver’s license when driving for any vehicle for work-related reasons.

CMA or CPhT preferred

Strong interpersonal skills

Basic understanding of Microsoft Excel
