They serve a bilingual, community-oriented patient base through an in-person clinical model with extended weekly hours.

✍ Short Description:
We’re seeking a Patient Liaison to support patient engagement and care coordination. This role involves contacting patients to follow up on appointments, lab work, and preventive care, ensuring care gaps are closed, and coordinating with clinical and administrative teams. The ideal candidate is detail-oriented, organized, and comfortable communicating with patients, providers, and internal staff to deliver a smooth and supportive patient experience.

💼 Responsibilities:
Maintain and update spreadsheets to track patients due for labs, physicals, and insurance gaps, specifically for PPO insurances

Contact patients via calls or texts to close overdue labs or care gaps

Coordinate with laboratories to submit lab requests and ensure timely receipt of results

Monitor quarterly performance goals and proactively engage patients to close care gaps

Schedule and support annual physical exams and preventive care services

Track and assist with insurance gaps to prevent delays in care delivery

Scan and distribute daily incoming faxes, reports, and pharmacy requests

Follow up on medical records requests and track Diabetic self-pay patients

Report positive Quantiferon results to relevant portals

Track radiology reports and follow up with imaging centers

Additional clerical duties as assigned

✅ Qualifications (Must Haves):
High school diploma or GED required; some related experience preferred

Strong communication skills for interacting with patients, coworkers, and management

Basic mathematical skills to compute rates, percentages, and interpret data

Ability to read and interpret documents such as safety rules and procedures

Detail-oriented with strong organization and multitasking skills

Familiarity with CRM and marketing tools, spreadsheets, and word processing software

Ability to apply common-sense reasoning to routine problems

Bilingual (English/Spanish)

Experience in healthcare or patient engagement roles

Knowledge of digital marketing, email campaigns, or social media in a healthcare context
