Experience Desired: Medication Therapy Management (MTM) experience, preferably using the Outcomes MTM Connect platform (1+ yrs); Call center or telephonic clinical experience (1+ yrs); Working with Microsoft Office and clinical documentation systems (1+ yrs)

Required Minimum Education: Bachelor’s Degree

Required Certifications/Licenses: Active pharmacist license in good standing in one of the following states: HI, TX, IL, FL, or PA

Job Description

Required Education:

• Bachelor’s degree in pharmacy or PharmD from an accredited School of Pharmacy

Required License/Certification:

• Active pharmacist license in good standing

• Must reside and be licensed in HI, TX, IL, FL, or PA

Required Qualifications / Skills / Experience:

• Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.

• Basic computer skills and proficiency with Microsoft Office products.

• Strong organizational skills, interpersonal skills, and detail-oriented.

• Experience maintaining focus and meeting goals in a work-from-home environment.

• Previous call center experience

Pharmacist - Medication Therapy Management (MTM) Overview:

• Medication Therapy Management supports the company vision to help people on their path to better health by performing MTM for members at the highest risk of having medication-related problems

• The team of pharmacists, technicians, and coordinators works together to identify problems and coordinate care to facilitate resolution to ensure each member is on an optimized medication regimen to help them reach their health-related goals.

Job Duties:

• Perform telephonic comprehensive medication reviews, including medication reconciliation and clinical assessments for targeted members

• Identify, document, and help resolve medication-related problems

• Create written patient summaries at an appropriate health-literacy level

• Prepare faxed materials for prescriber offices to resolve medication-related concerns

• Build and maintain positive relationships with internal and external partners, such as physicians and nurses

• Perform other responsibilities as directed by supervisor or manager
