We are seeking a talented and experienced MLTC Case Manager who is bilingual in English and Russian to join our team. The successful candidate will be responsible for providing remote case management services to our members. The MLTC Case Manager will work with interdisciplinary teams to ensure that our members receive the appropriate care and support needed to maintain their health and well-being.

Job Details
Bachelor's degree in nursing, social work, or a related field
Active NY State License
2+ years of experience in case management, preferably in a managed long-term care setting
Bilingual in English and Russian Preferred
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Understanding of state and federal regulations related to managed long-term care
Strong problem-solving and decision-making skills
Ability to prioritize and manage multiple tasks simultaneously
