Job Summary
As a Home Health RN with Interim HealthCare, you will have the opportunity to work in a fully remote role, with the flexibility to create your own schedule and enjoy a healthy work-life balance. You will provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehab, or nursing facility. Your responsibilities will include:
- Conducting patient assessments, documenting progress, and reporting changes to physicians
- Developing and implementing patient care plans, including education on prescribed medication, therapy, diet, and exercise
- Coaching family members on patient care, medication, and home safety
- Coordinating discharge from services and ensuring goals are met
- Working with a team of physicians and overseeing LPNs, CNAs, and aides providing patient care
Key Responsibilities
