Remote Nutritionist/Dietician-Spanish Bilingual

We are seeking a New York State licensed Nutritionist/Dietitian to join our Social Care Network program. This role focuses on providing nutrition assessments and individualized menu planning for two key populations: pregnant women and individuals with special needs. Your expertise will help improve health outcomes and support holistic care for vulnerable communities.

Key Responsibilities

· Conduct comprehensive nutrition assessments for program participants.

· Develop personalized meal plans tailored to medical, cultural, and lifestyle needs.

· Collaborate with healthcare providers, social workers, and program coordinators to ensure integrated care.

· Provide education and counseling on healthy eating habits and nutritional requirements.

· Maintain accurate documentation in compliance with program and state guidelines.

Qualifications

· Current NYS license as a Nutritionist or Registered Dietitian.

· Experience working with pregnant women and/or special needs populations preferred.

· Strong knowledge of Medicaid guidelines and community health programs.

· Excellent communication and cultural competency skills.

Pay: $40.00 - $48.00 per hour
