Senior Social Worker - Remote Hospice Care

Role Overview

Uplift Sobriety Counseling Solutions is seeking a dedicated and compassionate Senior Social Worker to join our remote team. This role is focused on providing essential support and counseling services within a hospice center setting, helping clients navigate their emotional and practical needs during challenging times. Key Responsibilities
• Conduct comprehensive assessments of clients' psychological, social, and emotional needs.
• Develop and implement individualized care plans in collaboration with multidisciplinary teams.
• Provide counseling and support to clients and their families, addressing grief and coping mechanisms.
• Facilitate support groups and educational workshops on topics related to end-of-life care and emotional wellness.
• Coordinate with healthcare providers to ensure holistic care for clients.
• Maintain accurate documentation and records in compliance with regulatory standards.

Required Qualifications
• Master's degree in Social Work (MSW) from an accredited institution.
• Current state licensure as a Clinical Social Worker.
• Minimum of 3 years of experience in a clinical or hospice setting.
• Strong interpersonal and communication skills.
