Telephonic Nurse Case Manager

Role Description
1. Comprehensive Case Assessment- Conduct thorough assessments to identify the unique needs of injured workers and develop tailored action plans to support their recovery and return to work.
2. Telephonic Support- Provide expert guidance, coordination, and support exclusively via phone to ensure appropriate and timely care delivery.
3. Resource Coordination- Connect injured workers with the right resources, bridging gaps between services and support networks to facilitate recovery
4. Licensed Professionals- Ensure all registered nurses hold a Compact License, maintaining compliance and professionalism.
5. Administrative Efficiency- Provide administrative support for referral assignments with a 3-day turnaround time to ensure prompt service delivery.
6. Caseload Oversight- Manage and oversee caseloads to meet 5-day memo and monthly reporting guidelines, ensuring accountability and timely communication
7. Technology Expertise- Utilize multiple case management software products, including ICMS, to streamline processes and enhance efficiency.
Timely Initial Contact- Ensure initial 3–4-point contact with injured workers within the specified timeframe to initiate case management promptly.
Must be Registered Nurse (RN)
Certifications- Certified Case Manager (CCM)- Certified Rehabilitation Registered Nurse (CRRN), Worker Compensation Experience a plus.
