We're seeking a dedicated and empathetic Registered Nurse (RN) Case Manager to deliver exceptional patient care in a home health setting. As a valued member of our team, you will play a vital role in coordinating and integrating healthcare services for our patients, ensuring seamless transitions and optimal outcomes.

About the Role:

• Full-time, day shift position with a competitive salary range of $34.00 - $51.00 per hour
• 40 hours per week, with occasional weekends and evenings required
• Remote work arrangement with travel to patient homes in the Hays, KS area
• Opportunity to work with a diverse patient population, including neonatal, pediatric, adolescent, adult, and geriatric patients

Key Responsibilities:

• Integrate healthcare services for patients from pre-admission to post-discharge, ensuring coordinated and sequenced care
• Evaluate patient outcomes using established clinical, financial, and utilization data
• Utilize independent nursing judgment to make informed decisions and develop personalized care plans
• Communicate effectively with patients, families, and interdisciplinary teams to promote collaboration and patient-centered care
• Document patient information accurately and efficiently, using electronic medical records

Requirements:

• Bachelor's degree in nursing preferred, with a minimum of one to two years of professional nursing experience in a clinic or home health setting
• Current, unencumbered RN license with the State Board of Nursing, or multistate licensure if in a Nurse Licensure Compact (NLC) state
• Strong knowledge of specialty area, with in-depth understanding of clinical pathways, guidelines, and standards of care
• Excellent communication, problem-solving, and critical thinking skills, with ability to work independently and as part of a team
